The scale of the Medicaid fraud is too big for DOJ to handle with traditional prosecutions. Unless Medicaid is abolished all of the fraud hunting will be just a fun exercise for data nerds. Abolish Medicaid, deport all foreign recipients and all foreign Medicaid profiteers.
